The Organization Prairie State Legal Services, Inc. is a not-for-profit legal aid organization that provides civil legal aid services to the poor, elderly, and people with disabilities. Prairie State has 12 offices serving 36 counties throughout northern and central Illinois. We focus services on legal problems that impact the ability of our clients to meet their basic human needs, including physical safety, access to healthcare, adequate housing, and similar needs. Prairie State is the only legal aid organization in the majority of our service area. Annually, we handle over 22,000 applications for legal help and serve over 16,000 clients. More information is available on our website at www.pslegal.org Job Description Prairie State Legal Services received grant funding to focus legal services on communities impacted by incarceration. We are seeking a temporary, full-time paralegal for our Rockford, IL office. The Paralegal will assist project attorneys with providing legal services to low-income individuals to resolve legal problems primarily focused on criminal records relief and drivers’ license issues and assist project attorneys with other legal services for such clients. Responsibilities Among other duties as assigned, the Paralegal will work under the supervision of attorneys: • Perform a high volume of document preparation and research, specifically related to charting criminal records so attorneys may evaluate criminal records relief options • Maintain contact with clients to provide information on the status of their cases • Type pleadings and correspondence for the project attorneys • Monitor case files to see that they contain all required documents • Generate and mail notices relating to court proceedings and matters to go on the court docket • Assist in coordination and execution of outreach clinics • Provide accurate and complete information for grant reporting to grant funders including documenting services in the case record • Assist project attorneys as directed. • Exhibit a high level of personal energy, organization, and commitment to the position and the PSLS mission Salary and Benefits This is a temporary position expected to last through June 30, 2020. The regular schedule is 37.5 hours/week with paid holidays. PSLS will conduct training for temporary staff hired under this project and such training may require travel within a few hours of the office. Travel costs are reimbursable.

RAMP's TBI Case Manager provides monthly case management to consumers who are found eligible for the State of Illinois' TBI Waiver Program. The State Department of Rehabilitation, Home Services Program staff determines eligibility and if the consumer requires monthly case management services, they make the referral to RAMP. The caseload ranges from 45-55 consumers. The majority of these consumers reside in Winnebago and DeKalb counties and a few in Boone and Stephenson County. Most consumers require a face to face home visit once a year at minimum. If their TBI significant impairs their cognitive function, these home visits will occur every 6 months. On the months that there is not a home visit, the case manager makes contact one on one with the consumer via the phone. RAMP's role is to ensure these consumers have a safe service plan and are connected with the proper resource in the community to ensure they are able to remain independent in the community versus an institution. The TBI Case Manager develops methods to ensure each person is contacted minimally every 30 days and documentation for each contact is completed within 2 business days. If during the monthly contact, there is a need or goal identified, the TBI Case Manager will assist the consumer in meeting this need. Therefore, some consumers will have more frequent contact than monthly. Additional services that a TBI Case Manager may provide to consumers on an as needed basis include: Advocating for home modifications or assistive devices Assisting the local Home Services office in obtaining bids for home mods and assistive devices Helping consumers recruit, interview, hire, manage and/or fire their Personal Assistant if they require our assistance. Providing referrals to community organizations for goals related to employment, housing, food assistance, medical assistance, etc. Providing peer support and referrals for counseling or support groups. **Diverse applicants are encouraged to apply
